OUR APPLICATION PROCESS CONSISTS OF THREE OPTIONS:

OPTION 1 (Application and Video):

a) Complete this application form (which will include a headshot and full body shot of yourself and a photocopy of your driver's license or government issued photo ID).
b) Send in your Application and Video a video (if you cannot attend an open call). Instructions for the video and address of where to send this to are available below or can be downloaded at: Click Here to Download the Application and Video Submission Instructions.

NOTE: YOU ONLY NEED TO SEND IN A VIDEO IN CONNECTION WITH YOUR APPLICATION IF YOU CAN NOT MAKE IT TO ONE OF THE OPEN CASTING CALLS DURING NOVEMBER OF 2009 OR DECEMBER 2009 IN ONE OF THE FOLLOWING CITIES: CHICAGO, DALLAS, LOS ANGELES, AND NEW YORK

OPTION 2 (Application and Open Casting Call):

a) Complete this application form (which will include a headshot and full body shot of yourself and a photocopy of your driver's license or government issued photo ID).
b) If you do not submit a video, you MUST attend one of our open casting calls. Dates, times, locations and what you need to bring for the open casting calls are available below. (Don’t forget to bring a signed original completed application, a photo of yourself and a photocopy of your driver's license or government issued Photo ID).

IMPORTANT INFO:

You must be at least 18 years of age as of the date you sign this application. If you want to apply for the show, You MUST bring the following items with you to the open call.


1. Completed and signed copy of this application (which includes a photocopy of your driver’s license or government issued photo ID);
2. A few, recent clear photos of yourself (a headshot and a couple full body shots from different angles)
3. Please come camera ready, in dance attire and don't forget to wear appropriate footwear. Most importantly, be ready to dance!

NOTE: YOU DO NOT NEED TO PREPARE A DANCE ROUTINE. WE WILL PROVIDE MUSIC FOR YOU TO DANCE. WE JUST WANT TO MEET YOU AND SEE YOUR BEST DANCE MOVES!

OPTION 3 (Email Us):

Please include your:

1. Contact info (don’t forget to include your name, age, email and phone number)
2. City you live in
3. Include a few recent photos of yourself (a headshot and a couple body shots from different angles)
4. A paragraph about why you want to be on this show. Also, be sure to include a brief explanation about why you’re finally ready drop the weight and pop, lock and drop it like it’s hot!

No Application will be complete until we receive the signed original by video submission or in person at an open call.

SEND US YOUR COMPLETED APPLICATION AND VIDEO

3. Your videotape (no longer than 10 minutes in duration) clearly labeled with your name, address, email, and phone number.

TAPE DEADLINE EXTENDED! We will be accepting home tapes until January 4th, 2010.

DEADLINE: Completed applications and videotapes must be received as soon as possible or before December 11th, 2009 at 12:00 PM ET to be considered.* Remember, the earlier the better.
